Alcohol, I think is more painful than smoking. But then again, respiratory disease, which is caused by smoking or, is one of the leading causes of death in the U.S. And then again, alcohol is one of the leading causes of death for teens due to accidents. I don't know which is worse, really.
I have to go with smoking. With Alcohol you can drink a lot one night and puke the next morning and boom it's out of your system. With smoking every puff starts to eat away at your lungs and in the next 2 decades you could die from lung cancer. But I think a little amount of Alcohol can actually be good for you every once in a while. So smoking is worse.

1: alcohol; Correction, when you puke, its still in your system. It still destroys your liver, makes you develop that sense that you have to drink more to get "buzzed", and impairs practically every sense you have. Also, if youre taking medications and drinking, it can kill you within minutes. Oh, and no, the saying is a little red wine every once in a while is good for your heart, but its replaceable with grape juice. Even then, ANY alcohol in your system is a bad thing, since it can quickly and efficiently destroy your liver which would lead to everything else failing within a quicker time period than smoking.
2: smoking; although it does cause lung cancer, and can give you an addiction depending on what youre smoking, it has no noticeable effects for a while, whereas alcohol can kill you the first drink. Smoking typically wont kill you quickly unless you have a major heart or lung problem. It does cause youre teeth to yellow, so its easier to tell if someone smokes, but it has no impairments (unless its an illegal substance, but those impairments take more to take effect than alcohol.) that occur on the spot but addiction. The rest happens over time.
