Two quick reply sections?

Why are there two quick reply sections when viewing a thread? I assume this has to do with a template edit being done twice? Can an admin fix this? It looks really bad and confused me the first time I tried replying to a thread.

There's supposed to be a quick reply section under the existing posts in the thread, but where does the 2nd one appear for you?

Hey Smokey, I am not sure what you mean? There is only one quick reply section for me. If you could take a screenshot of this error, it would be greatly appreciated. Also, if anyone else has this issue or has had it before, please let me know.
